German Shepherd Hip Dysplasia: Signs, Screening and the One Thing Owners Mistake It For
German Shepherds have one of the highest rates of hip dysplasia of any breed, and the situation is better than it used to be. In Switzerland, where hip screening data has been collected on the breed since the mid nineties, the proportion of screened German Shepherds classified as dysplastic fell from 46.2 percent to 18.0 percent over roughly two decades. Selection works. It has just not finished working.
But before you read any further about hips, there is a fork in the road that matters more for this breed than for almost any other, and getting it wrong costs owners months.
First: Is It the Hips, or Is It the Spinal Cord?
The German Shepherd is the breed most associated with degenerative myelopathy, a progressive disease of the spinal cord. Merck lists it as most common in German Shepherd Dogs, and UC Davis says it has been reported most commonly in German Shepherds and German Shepherd mixes.
Early on, the two look alike from across the room. A back end that is not working properly. Trouble on stairs. A dog that used to leap into the car and now does not. VCA puts it plainly: in its early stages the signs of degenerative myelopathy resemble those of arthritis, which often follows hip dysplasia in large breeds, which makes the diagnosis genuinely difficult.
They are not, however, the same problem, and the differences are things you can observe at home before you ever get to a vet.

The single most useful line in the veterinary literature on this comes from Merck: early cases of degenerative myelopathy may be confused with orthopaedic disorders, but proprioceptive deficits are an early feature of degenerative myelopathy and are not evident in orthopaedic disease. In plain terms, a hip dysplasia dog knows exactly where its feet are and it hurts to use them. A degenerative myelopathy dog has stopped knowing, and it does not hurt.
That single sentence is worth carrying into the consulting room, because it is the distinction the whole diagnosis turns on.
Two more things worth knowing before the appointment. There is a genetic test for the SOD1 variant, and the honest reading of it is narrower than most people assume: the University of Missouri describes a dog with two copies as at risk, and states that not all dogs with the mutation develop the disease, so the test is currently a test for risk rather than a diagnosis. And degenerative myelopathy is a diagnosis of exclusion. Cornell notes there is no specific test to diagnose it in a living patient, which is precisely why ruling the hips in or out properly is step one.
Which means the honest sequence is to rule the hips in or out first, and to treat a genetic result as a risk figure rather than an answer.
This matters commercially as well as clinically. Degenerative myelopathy is the condition most German Shepherds in a dog wheelchair for back legs actually have. Hip dysplasia rarely puts a dog in wheels, and putting an under medicated painful dog into a cart makes things worse rather than better.

What Hip Dysplasia Looks Like in a German Shepherd
Assuming it is the hips, the signs are the breed neutral ones, and the German Shepherd specific problem is that this breed is expected to look like that.
A sloping rear, a back end that sits low, a dog that is a bit lazy about stairs, a dog that lies down in a splayed frog position. Owners of other breeds notice these things. German Shepherd owners have been told for decades that this is what the breed looks like.
So the useful signs for this breed are the ones about change rather than appearance. A dog that used to sit square and now sits with a leg kicked out. A dog that used to take the stairs one at a time and now hops them with both back legs together. A dog that stopped getting on the sofa this year. Stiffness in the first few minutes after a nap that walks off. A stance that has narrowed at the hocks since last summer.

What separates a dog that is simply built low from a dog whose hips are failing is change over time, not a snapshot.

The Sloped Back Question
Every German Shepherd forum has this argument, so here is what the evidence actually supports, including the part that is unresolved.
The change in the breed is real and documented. A 2020 review of working dog structure notes that German Shepherds, particularly those bred for conformation shows, have shown marked increases in rear limb angulation from moderate to extremely angulated over the last several decades.
The biomechanics change with it. A separate 2020 study measuring German Shepherds of differing back slope found asymmetry in stifle flexion, hock flexion and hock adduction increasing with the slope.
What has not been shown is the causal step everyone assumes. The authors of that biomechanics study screened all their dogs clear of hip dysplasia and concluded that further studies are required to determine whether the variation in conformation relates to increased susceptibility to musculoskeletal disorders. The working dog review is equally careful: it suggests the angulation may reflect unintended selection for greater extensibility of tendons and ligaments generally, and calls it perhaps not surprising that the breed has a very high prevalence of hip dysplasia compared with other large breeds. That is a hypothesis stated as one, and the same paper says outright that the effects of the extremely sloped topline have not been objectively studied.
The honest position: the topline changed, the way these dogs move changed with it, the breed has a high prevalence of hip dysplasia, and nobody has yet demonstrated that the first thing causes the third. Buy on documented hips rather than on topline, because hips are the thing that has been measured.
The One Screening Fact That Is Specific to This Breed
If you take one technical thing from this page, take this one, because it changes what a screening result means for a German Shepherd.
PennHIP measures hip laxity as a distraction index, and there is a rough threshold at around 0.30 below which hips have very low susceptibility to osteoarthritis. But the risk curve is not the same for every breed. PennHIP's own material notes a shift for the German Shepherd Dog, indicating increased risk of expressing osteoarthritis at a given distraction index compared with other breeds.
Read that again in practical terms. A laxity measurement that would be reassuring in a Labrador is less reassuring in a German Shepherd. The same number carries more risk in this breed. It is an argument for screening earlier and for taking a middling result more seriously than the number alone suggests.
Timing is the other half of it. PennHIP can be done from sixteen weeks. The Orthopedic Foundation for Animals does not issue final certification until twenty four months, and the surgical procedures that can still change the shape of a hip have to happen far earlier than that. Waiting for the certificate means waiting past the window.
Neutering Timing, Which Is a Real Variable Here
This one comes up constantly with German Shepherds and deserves to be reported accurately rather than turned into a rule.
In a study of roughly fifteen years of records from the UC Davis veterinary teaching hospital, German Shepherd females neutered before six months were diagnosed with joint disorders at around 20 percent, and those neutered between six and eleven months at around 15 percent, against roughly 5 percent of intact females.
Three caveats, all of which matter. These are teaching hospital records, which carry referral bias. Findings differ by breed and by sex, and cannot be extrapolated from one breed to another. And the decision sits against real counterweights including cancer risk, pyometra and population control. Bring the numbers to your vet. Do not let a retailer tell you when to neuter your dog, including this one.
What Actually Helps a German Shepherd With Bad Hips
The management is the same as for any breed, and the ranking is not what most owners expect.
Weight is first and it is not close. A lifetime study of Labradors fed 25 percent less than their littermates found 14 percent had hip arthritis at eight years against 64 percent of the normally fed group, and the leaner dogs lived on average 1.8 years longer. Nothing sold in a bottle comes close to that effect size.
Exercise second, and the shape of it matters more than the amount. Little and often, on surfaces with grip. Swimming and walking rather than fetch with hard turns. The pattern to avoid is sedentary all week and a long hike on Sunday.
Pain relief third, and it is a prescription conversation. There are several NSAIDs licensed for dogs, they need periodic blood monitoring, and there are newer options including a monthly injection that blocks nerve growth factor.
Supplements last, and honestly. Omega-3 from fish oil has the best support in the literature. Glucosamine and chondroitin are where popular advice and veterinary guidance part company: the American College of Veterinary Surgeons states there is no evidence in peer reviewed literature that cartilage protective supplements repair cartilage or protect it from wear.
Ramps and non slip flooring are unglamorous and they remove the single worst load on a bad hip, which is the impact of jumping down.

When Wheels Come Into It

A cart substitutes for a back end that cannot carry weight. That describes a degenerative myelopathy dog, and it is why the breed is so heavily represented among wheelchair users. It rarely describes a hip dysplasia dog, whose legs work but hurt, and who needs pain control and weight loss before equipment.
Where support helps a hip dysplasia dog earlier is narrower and more specific: a sling or rear support harness for stairs and car entry, because those are the individual movements that hurt, rather than a cart for general mobility.

Buying a German Shepherd Puppy
Given the prevalence numbers at the top of this page, this is where most of the outcome is decided.
Ask for documented hip results on both parents, and then ask the question most buyers do not: what were the results on previous litters and on siblings. Progeny data tells you more than one dog's grade does.
Ask whether the line has been PennHIP tested rather than only OFA graded, given what the breed specific risk curve does to the interpretation of a laxity score.
Then take the two decisions that are entirely yours. Feed measured meals rather than free choice, because in one study two thirds of free fed puppies developed hip dysplasia against one third fed measured amounts. And use a large breed puppy food without supplementing calcium, because over supplementing calcium in a growing large breed puppy causes developmental orthopaedic disease.
