    A rear wheelchair suits a dog whose back legs have failed but whose front end is still strong, and whose weakness is not being caused by pain. Before buying one, the more important step is finding out why the legs are going, because some causes are treatable, some are reversible, and a wheelchair fitted too early can get in the way of a dog who might have walked again.

    That is not the usual advice on this subject, so it is worth saying plainly what the evidence actually is. There is almost none. The first study of assistive carts in veterinary medicine was published in 2024, and its authors wrote that no previous studies existed. It found real benefits and a complication rate in dogs of 60 percent.

    Both halves of that matter. This page covers what is known, what is not, and how to have a useful conversation with your vet.

    Start With Why, Not What to Buy

    Back legs fail for a dozen different reasons, and the single most useful thing an owner can establish early is whether the dog is in pain.

    Establishing pain sorts the causes into two groups that need completely different responses. A dog whose problem is purely neurological, where the signal is not reaching the legs, is the classic wheelchair case: the legs do not work, but nothing hurts. A dog whose problem is pain is limited by the pain, and a cart does not treat pain. Getting more activity out of a painful dog without addressing the pain first can make things worse.

    Look at the right hand column. Three of those six can improve or resolve. That is the reason not to start with the shopping.

    One thing worth knowing if a vet mentions it: spondylosis, the bony spurs that show up along the spine on x-rays, is present in a large share of dogs by the age of nine and usually has nothing to do with the symptoms. It is commonly found and rarely the explanation.

    The One Thing Your Vet Will Check First

    For any spinal cause, the strongest predictor of whether a dog walks again is whether they still feel deep pain in the affected legs. It is a specific test and it is not the same as the leg twitching when touched.

    The numbers are stark. For a small dog with a disc extrusion who cannot walk but still has deep pain sensation, surgical recovery rates run around 85 to 95 percent. If deep pain has been absent for more than a day, it drops below half.

    This is worth understanding before you buy anything, because it tells you which situation you are in.

    What the Research on Wheelchairs Actually Says

    Very little, and being straight about that is more useful than pretending otherwise.

    The one study is a 2024 survey of over 1,200 owners of animals using mobility carts. In dogs it found that 62 percent of owners reported improved quality of life for the animal, 61 percent for themselves, and around 79 percent said they would recommend cart use. Play, eating and drinking, and walking all improved for the majority.

    It also found a complication rate of 60 percent in dogs, and just over half of those complications were wounds.

    That study is self reported and retrospective, with obvious selection bias, so treat both numbers as indicative rather than clinical. But an owner deciding this should know both, and most pages selling wheelchairs mention only the first.

    When a Cart Helps, and When It Gets in the Way

    Every veterinary source that addresses this routes the decision through a vet, and so do we. What we can usefully do is tell you what the concerns are, so the conversation is a better one.

    The situations where carts are generally discussed are severe arthritis including hip and elbow dysplasia, neuromuscular disease causing limb weakness, spinal or neurologic paralysis, recovery from surgery or amputation, and mobility loss after an accident.

    The caution that almost never appears in retail copy comes from Darryl Millis, one of the founders of canine rehabilitation, writing in a standard veterinary text: "Carts should not be used too early in the course of rehabilitation because dogs may become too dependent on their support."

    That is expert opinion rather than experimental evidence, and no one has studied it directly. But there is a reason to take it seriously, which is how long recovery can take. In dogs who lost deep pain sensation after acute spinal injury, studies have found somewhere between 10 and 59 percent eventually walk again, and in one group the recovery took an average of nine months, with a range out to eighteen. In another, 58 percent of paraplegic dogs given daily physiotherapy with standing support regained a form of walking, over 25 to 64 weeks.

    Note what those dogs were given: slings, harnesses, frames, support that held them standing and weight bearing. That is a different intervention from a cart that carries the hind end while the legs hang. Whether the two are interchangeable has not been studied, which is exactly why it belongs in a conversation with the vet rather than in an ad.

    Rear Support or Full Support

    Clinically the trigger for a four wheel cart is front limb involvement, not dog size and not severity in general.

    Dogs carry roughly 60 percent of their weight on their front legs already, which is part of why most adapt to a rear cart reasonably well: nothing new is being asked of the front end. In the 2024 survey, dogs in rear wheel carts showed better quality of life scores than dogs in four wheel or front wheel carts, though that is very likely because dogs needing a quad cart are sicker to begin with.

    The important warning is the other way round. If your dog is already in a rear cart and the front legs start to weaken, that is not a signal to upgrade the cart. It is a signal to go back to the vet, because in progressive disease it can mean the condition has advanced, and in other cases it can mean a second problem.

    Fit, and the Complications to Watch For

    Since more than half the reported complications were wounds, it is worth knowing where they occur. In that survey the most common sites were the inside of the hind thigh at around 28 percent, the inside of the front leg and armpit at 22 percent, and the tops of the paws at 14 percent, followed by the belly, back, and neck.

    The paw figure points at something specific: dogs whose back feet drag or knuckle under while in the cart wear through the top of the foot. Boots or a sling support that keeps the feet off the ground is the usual answer.

    The thigh and armpit figures are a padding and adjustment problem. It is the reason our wheelchair uses a cushioned belly sling and padded rear leg rings rather than bare webbing at those contact points, and why it adjusts in width as well as height and length. A frame that only adjusts in two dimensions will sit wrong on a narrow or a barrel chested dog no matter how carefully you set it.

    What fit problems look like day to day:

    • Rear set too low. Hips sink, the frame catches on thresholds, and the paws drag more.
    • Rear set too high. The dog pitches forward and the front paws slap down hard.
    • Balance off. Starting feels abrupt, as though the cart pulls back or shoves forward.
    • Early rubbing. Fur parts, damp patches appear, or the dog flinches while being harnessed.
    • Tracking off. The rear swings wide, one side scuffs, or it drifts on a straight line.

    Change one variable at a time and retest on the same short stretch. Set the height so the body looks level, then the balance so starts feel smooth, then the strap tension. Changing all three at once is the most common mistake, and it leaves you with no idea what helped.

    A sudden refusal to move is usually a comfort problem rather than stubbornness. Check the height and the balance before you conclude anything about the dog's attitude.

    Three rules from the veterinary literature, and they are not fine print.

    Supervise every session. Dogs in carts fall down stairs and tip over.

    Keep sessions short enough to avoid fatigue. Nobody has established a safe maximum, so any specific number of minutes you see quoted online was invented. Your vet or rehab practitioner sets it for your dog.

    Check the skin every single day, particularly the inner thighs, the armpits and the tops of the feet.

    The Care That Comes With It

    A cart addresses movement. It does not address the rest, and the rest is usually the harder part.

    Many dogs with hind limb paralysis cannot empty their bladder on their own and need it expressed manually, several times a day. This has to be taught hands on by a vet, because doing it badly leaves urine behind, and retained urine is how infections start. Urinary tract infections are reported in something like 27 to 42 percent of recumbent and spinal patients, so it is not a remote risk.

    Signs to act on: strong smelling, cloudy or bloody urine, straining, a bladder that feels hard and will not express, reduced output, new incontinence, fever, or pain when the belly is handled.

    Skin is the other daily job. Urine against skin causes scalding, dragging causes abrasions, and pressure sores form over bony points in dogs who spend a lot of time lying down. Keeping the dog clean and dry does more than any product.

    One design detail matters more here than it sounds. A cart the dog has to be lifted out of to toilet turns every bathroom trip into a two person job and leaves urine sitting on the frame and the coat. Ours is built open at the rear so a dog can relieve themselves without coming out of the frame, which is a small thing on a product page and a large one at six in the morning.

    The Conversation Nobody Puts on a Product Page

    If the diagnosis is degenerative myelopathy, some honesty is more use than encouragement.

    It is not painful, which is genuinely good news. It is also relentless. Most dogs become unable to walk somewhere between six and twelve months from diagnosis, and no treatment has been shown to change that. One small 2006 study of 22 dogs found longer survival among those receiving daily physiotherapy. It has never been replicated, the dogs were not confirmed cases, and owners chose their own group, so it may partly be measuring how committed those owners were.

    Most dogs with this diagnosis are put to sleep when they stop being able to walk or become incontinent, rather than at the end of the disease. In a cohort that was carried through to the end with full nursing care, the disease progressed past the back legs to the front, and 85 percent developed breathing difficulty, after which median survival was about six weeks.

    A wheelchair changes what the middle of that looks like, and for a lot of dogs the middle is a good stretch of life. It does not change where it goes.     Your own capacity is a legitimate part of this. Whether you can lift the dog, whether you can be home every four to six hours, what your house is laid out like, what it costs. Those are not selfish considerations. They are part of what the dog's daily life will actually be.
